additional fields when applying for position

Currently the client is using the “Link to the PCRecruiter Web Extensions directly.” option and in the screen where a registered user applies for the position they are shown some options including;
1. Preferred contact method?
2. Enter Any Additional Comments?
3. Forward a Copy of your resume?
4. include contact information?
Can we replicate these using the rest API?
We see there is an option to add notes so that takes care of number 3. just leaving us with 1. 2. and 4.

Any clues to push us in the right direction would be appreciated.

I did not find the right solution from the internet.
